Circle a birdie, square a bogey. Front nine totals in OUT, back nine in IN.
How to Save It as a PDF
There is no separate download button because there does not need to be one. Press the print button, and in the dialog that opens choose Save as PDF instead of a printer. You get the same file, without handing over an email address for it.
One thing worth checking: set the orientation to landscape. An 18-hole card is 21 columns across and portrait paper will squeeze it until the boxes are too small to write in.
Marking the Card
These four marks are close to universal, and they let anyone read a card without adding a single number up.
Birdie
One under par. Circle it.
Eagle
Two under. Two circles.
Bogey
One over par. Square it.
Double bogey
Two over. Two squares.

Filling In the Card Properly
In stroke play you mark a playing partner's card, not your own, and they mark yours. At the end you both check the hole scores, sign, and hand it in. Getting a hole score wrong on a card you signed is the classic way to be disqualified from a competition.
Totals are the marker's job to add but the committee's job to verify, so an arithmetic slip in the OUT box is not fatal. A wrong number in a single hole box is.
